I would like to create a web page on which I could possibly ask survey questions and have the responses submitted to me on my PC. Is there a free program that would allow me to do this and create my own unique URL go along with the page for free? If so what is the program and how does it work? For example, if I create a webpage with the program on which I ask questions, and people answer the questions, how will the answers be submitted to me? Will the answers become stored/saved somewhere?

you can NOT create a url. You need to subscribe to one with a domain registrar. This requires identity information being given for each registration. It can take 5 days for a domain name to appear, you then need to set the registrar's dns to point to your hosting site, this again requires subscribing. you will need to learn php and the mysql database (most used on the internet) and then store the data on this, and maybe use the php mail() function in its' 5 parameter form to send it to oyur email.
